PatrickDuring the course ofEMR implementation project at a large medical center, the issue of how best todesign display screens for the system arises. The particular concern is whatstandard codes should be used for the information on the user screens. Thefollowing e-mail is a request for a consultation from an IT staff involved in theEMR project to a member of the health informatics group:Carol,I need your help withsomething, and it s a pretty big something. I wonder if you or anyone on yourstaff would be interested in taking this on as a project?I m working on the newEMR project. One of the things we re stuck on right now is standard displays ofinformation in the EMR. Because the record is integrated (which is the goodnews and the bad news), most of the tables and code sets are shared amongmultiple disciplines. We need to define what we will use for our displays. Forexample, the code set we re working on right now is Units of Measure. Now, youwouldn t think it would be too hard to decide what the display for somethinglike milligrams would be. Except the choices are MG, Mg, mg, etc . Andthat s one of the easy ones.According to themedical center s standard abbreviations, all of the above are perfectlyacceptable. The problem is, to build the EMR, we have to decide on just one. Sofar, since we began implementing the EMR, we ve included a mishmash ofdisplays, depending on what department we were working with. Lo and behold, nowwe re in a real pickle, and our database is a mess. In the truly integratedEMR, there can only by one abbreviation displayed for milligrams. Again, this is justone example out of literally hundreds (if not thousands) of pieces ofinformation we need to standardize.Do you have some timeto help us figure out what standards are out there? We d like to standardize onsomething that is nationally, if not internationally, accepted. It needs toinclude content for standards, definitions, and especially abbreviations forpharmacy, medicine, nursing, and purchasing units. We d like to have somestandards to present to the EMR Steering Committee and the Medical RecordsCommittee for consideration.Do you have anysuggestions?
