In this written assessment, students will formulate a plan of care attributed to a patient scenario using the Australian Commission on Safety and Quality in Health Care. This will allow the students to demonstrate their understanding of the evidence-based health care practices, coordination of patient care to improve patient outcomes and associated pathophysiology, development of patient-centred goals, and interventions of care. This will, in turn, enable students to be clinically competent in nursing practice. ASSESSMENT OVERVIEW An accurate word count including in-text citations, excluding the reference list. Part A: This is an individual assessment task based on a patient clinical scenario where students are expected to use clinical reasoning and critical thinking to apply the evidence-based nursing practice to develop a plan of patient-centred care. 1200 words 35% Part B: Group presentation to demonstrate competency in effective patient-centred care on discharge. This section follows on from Part A individual written assessment. Key Assessment Information PURPOSE The purpose of this assessment task is to: Apply evidence-based nursing practice using national standards with an understanding of pathophysiological concepts to provide patient-centred care. Learn to think like a nurse to assess, plan, implement and evaluate the treatment and management of patients with acute and chronic conditions. Develop clinical reasoning, critical thinking, and critical decision-making skills which are required for the provision of culturally sensitive quality patient care. Through national standards learn competent levels of patient care in each phase (admission, discharge) of the nursing process. Develop an understanding of how to utilise national standards for a given case scenario to develop clinical proficiency and safety. LEARNING OUTCOMES LO1: Integrate and corroborate evidence-based knowledge and legal and ethical considerations when making clinical decisions in relation to person-centred care scenarios across the lifespan with a range of deteriorating complex illnesses/conditions; LO 3: Exemplify an intraprofessional and interprofessional person-centred communication approach in comprehensive evidence-based assessment, and nursing interventions through simulated scenarios; LO 5: Exemplify the use of remote and in-person digital health approaches in communications to Aboriginal and Torres Strait Islander peoples, other cultures, and people of diversity when delivering complex nursing interventions. ASSESSMENT DETAILS The written assessment task is related to a case scenario that will be provided to students in week 1. The context of the scenario is based on Comprehensive patient care: Shock and basic life support/METCall covered in the first week of the Block. The assessment piece should be developed using the Australian Commission on Safety and Quality in Health Care standard. Evidence-based practice is applied to develop patient-centred care interventions using critical thinking and clinical reasoning to explore the patient’s history, presenting problems, and the related pathophysiological processes. With adherence to cultural awareness, the nursing standards, code of conduct, code of ethics, and legalities, a holistic care plan is developed by setting specific patient-centred goals and formulating care interventions. This is an academic assignment; therefore, academic standards inclusive of grammar, sentence structure, paraphrasing, and APA 7th edition referencing for both in-text citations and referencing list are required. Introduction, body, or conclusion is not required. INSTRUCTIONS : To get started on your assessment task, please follow the below instructions. Read through the case study and rubric for this assessment task. Rubric (Can be found under the Assessment Criteria below) Case Study: It is 0800hrs. Elliot, a 67-year-old Aboriginal man, came in yesterday with chest pain. ECG revealed acute ST-segment elevation (in leads V2-V6) myocardial infarction (STEMI). Elliot was transferred from the emergency department to Cath lab for an urgent angiogram and percutaneous coronary intervention (PCI) as he was at risk of cardiogenic shock. Day 1 post PCI, Elliot has an elevated Troponin (53 ng/L as of 0600 today). He feels tired today and is sitting out of bed as he states that it improves his breathing. Elliot has had a restless night, and at 0200hrs, began to desaturate (SpO2 92% RA). Subsequently, he was commenced on oxygen at 6l/min via Hudson mask. He removed the oxygen in the morning as he found the mask uncomfortable. His medical past history includes Hyperlipidemia, T2DM, and he smokes one packet of cigarettes in two days. He weighs 130kg, and his height is 140cm. After your initial safety check, you take his vital signs. Elliot\’s BP is 100/60 (the last reading was 115/70). His heart rate is 74 (slightly raised since the last reading when his heart rate was 64). Temperature 37.5. (from 36.8) His respiratory rate has increased to 24 breaths per minute (from 18 breaths per minute). You were caught up with the other two patients and did not get a chance to check up on Elliot. It is 1000hrs now, and you heard a METCall being announced for Elliot\’s room. As you entered his room, you noticed that he was confused, cyanotic, and looked lethargic. His BP is 86/50, and his HR is irregular and tachycardic. As the MET arrived, Elliot became unconscious, and you started cardiac compressions. Complete the Part A and Part B requirements below. Submit your work before the due date by following the submission instructions. Assessment 2: Part A Individual Written Assignment Refer to the NSQHS second edition for Recognising and Responding to Acute Deterioration Standard and other evidence-based resources/Standards to identify and explain the contributing factors for Elliot\’s deteriorating condition. (250words) Using the NSQHS second edition for Recognising and Responding to Acute Deterioration Standard and other evidence-based resources/Standards, identify and discuss four evidence-based prevention nursing strategies that can be implemented for Elliot during his hospital stay to prevent further complications. (600 words) Identify two different validated risk assessment tools that recognise patient deterioration with clear escalation processes embedded and discuss/critique the effectiveness with supporting evidence.
