John is a 56-year old male interested in starting an exercise program. He has had an
existing diagnosis of high blood pressure and high blood cholesterol for the past five
years (each) and is taking medication for both. John is also considered obese and meets
the criteria for the diagnosis of metabolic syndrome.
John has been sedentary for many years, does not smoke cigarettes, drinks occasional
alcohol and has a family history of premature heart disease. He is married, has two
grown children who are healthy, and works as a bank manager in town. He hopes that
through this exercise program he can lose weight and manage his high blood pressure
more effectively.
John sits at a desk most of the day at the bank and most of the time feels too tired to
exercise when he comes home. He used to be a member of a nearby gym when his
children were growing up but has not renewed his membership for several years. He
also used to enjoy playing softball with friends about 15 to 20 years ago. When the
weather is nice, John and his wife walk their dog in the neighborhood on the weekends.
But, most days they hire a dog walker since they each work about 10 hours a day. He
has friends who participate in golf leagues and running clubs, which he has considered
joining, but doesn’t’t think he has the time. He has also thought about doing weight
training but isn’t sure how to start such a program without injuring himself.
Currently, John does not participate in a purposeful exercise program. His daily activities
consist of about 60 minutes of weekend chores around the house and dog walking, but
essentially no activities throughout the rest of the week.
When asked the question, “On a scale from 1 to 10 (1 being the worst and 10 being the
best), how would you rate your overall weekly physical activity?” John answered with a
“3.” When asked, “Why did you not rate yourself as a “2”, he replied, “I do get some
exercise on the weekends.” Then, when asked, “What would you need to do to answer
the question with a “4?” he replied, “If I were able to take a walk around the
neighborhood at least 2 times per week on days other than the weekends, I would
consider myself as a “4.”
His physical activity goal is just to start exercising since he currently doesn’t have an
established routine. He wants to actually be able to maintain an exercise regimen that he
enjoys.
