Prepare Part of a Physician Credentialing Application Packet
The AvMed provider credentialing application form referred to in this chapter is quite lengthy. In addition to the form itself, there is a list of documents that must be attached with the application located toward the end. A screen shot of that section follows this assignment. Assume you are responsible for getting a new physician with your practice credentialed with AvMed. Research some physician profiles (many facilities and practices highlight their physicians’ credentials on their websites).
- Type up the below list of documents and for each one:
- describe what that document is for
- describe why you think it is important for the payer (in this case but similar for obtaining hospital privileges) to review that specific information.
Please submit an APA formatted Word document with the following information to this Dropbox by the designated deadline:

- Properly formatted reference page and remember to use in-text citations to support your work. Every in-text citation should be on the reference list. For this assignment, references are required for the description of the required documents.
