The assignment is your opportunity to critically evaluate the role of theory in conceptualization and practice. It is expected that you demonstrate diversity and cultural awareness as it applies to the individual cases. Treatment plans should include an overview of the case, a brief presentation of the theory, and a client specific plan for a beginning, miIDle and end of treatment with appropriate interventions. Students should offer a few examples of these interventions. In aIDition, it is expected that students will make use of assigned texts and other materials for references. A sample of what the professor is expecting is listed below. Please review the template below. Do not alter the main template headings. You are required to use this template for this assignment.
The Case Conceptualization Paper is designed to have the student select one Individual Theory and one Family Theoryof the following theories and apply these two theories to the critical review of the vignette below using the template below:

Imagine that you are working with the following client in therapy
Mike is a 19 year old African American male (he has two brothers and he is the youngest child). His parents brought him to therapy because Mike has been reportedly stressed. He is reported to worry for prolonged periods of time. His worrying has interfered with his grades in school. He cant focus and concentrate and he is failing several classes. His sleep has decreased because he worries about various issues (no particular topic). In therapy he reported that his symptoms of excessive worrying began about one year ago. He states, I feel like I cant go anywhere alone. I feel like I always have to go somewhere with someone. I feel like I depend on others too much but its my reality. Its like I need someone to protect me. I have lost relationships with good friends because they tell me that I am too needy. Mike mentions that if he goes places alone he starts to feel stressed and he describes the following symptoms: I feel nervous and scared, like something bad is going to happen, I feel my heart racing, my muscles get real tense and I get dizzy and nausea. I feel like I should just stay home and avoid going anywhere but I have to graduate high school. Mike reports he was working a part time job that he eventually quit because he kept calling in sick and avoiding work. His mom mentioned that now he is trying to avoid going to school and she is afraid he may not graduate. In the session he makes the following statements: If I go places alone, than I feel like something bad is going to happen. If I dont get my act together, I know my life is over. Why does everything feel like its a catastrophe. Mikes mom reported that he recently began drinking. Mike states, I drink because it makes me feel better, it makes the stress decrease and the worrying decrease for awhile.
