Prepare Part of a Physician Credentialing Application Packet

The AvMed provider credentialing application form referred to in this chapter is quite lengthy. In addition to the form itself, there is a list of documents that must be attached with the application located toward the end. A screen shot of that section follows this assignment. Assume you are responsible for getting a new physician with your practice credentialed with AvMed. Research some physician profiles (many facilities and practices highlight their physicians’ credentials on their websites).

• Type up the below list of documents and for each one:
  • describe what that document is for
  • describe why you think it is important for the payer (in this case but similar for obtaining hospital privileges) to review that specific information.
